PLEASE READ: The below information applies to any RFC member who is or will be doing any type of initial or recurrent flight training for the purpose of obtaining a license, permit or rating for COMMERCIAL INTENT ONLY. If you will only be renting a RFC aircraft and not doing any type of training with RFC, will be a non-flying member or is obtaining a license in the current year for recreational purposes only, then you do not need to fill out Section 1. If you are starting flight training, currently enrolled in a training program or anticipate you will at some point this year commence flight training for the purpose stated above, please read and fill in the entire form below.
SECTION 1: CRA TUITION CERTIFICATE: In 2019 Canada Revenue Agency changed its tuition reporting structure and it is now mandatory for educational institutes to collect and submit certain personal information for their students on the T2202 form. By signing below, I give permission to the Regina Flying Club to collect and submit my name, mailing address, tuition amounts and Social Insurance Number and any other required information on the T2202 form for the purpose of filling tuition tax forms for Canada Revenue Agency. A copy of this information will be submitted to Canada Revenue Agency and the student for the purpose of issuing Tuition and Enrolment Certificates. A copy will also be retained with the Regina Flying Club for the purpose of maintaining the appropriate records. I give the Regina Flying Club permission to submit the T2202 form to Canada Revenue Agency on my behalf but understand that it is my responsibility on an annual basis to confirm whether the T2202 tuition form applies to my training needs or not (it may not apply to everyone). I give permission for Regina Flying Club to send my copy of the annual T2202 tuition and enrolment certificate via the email address I provided to the Regina Flying Club on the membership form or an email address I update with RFC office staff. This emailed form will contain the information listed above and is used for the purpose of income tax returns.
SECTION 2: TRAINING AT ANOTHER SCHOOL(S)
In some certain circumstances it may be required that the Regina Flying Club needs to contact a student’s previous school to inquire about their flight training. I give permission for the Regina Flying Club to contact the above school(s) for the purpose of discussing the flight training I completed at the previous school(s) to aid in the seamless transition in training with one organization to another. Topics discussed will only pertain to the students performance during flight training which may include but not limited to performance on certain exercises; completion of flight times or ground school; tests and their grades required for the applicable licence, permit or rating; students aptitude and ability to learn new skills applicable towards operating an aircraft; any comments made in the student’s PTR regarding student performance; or flight safety. I also give permission for my previous flight school(s) to release this information to the Regina Flying Club as described above
SECTION 3: MODEL RELEASE FORM: As celebration and encouragement to our students, Regina Flying Club posts milestone achievements of our students on various social media sites. By signing below I give the Regina Flying Club permission to post, publish or use my name, course and any photos of me that may be taken for the purpose of, but not limited to first solo; completing license, permit or rating; or flight test; at a Regina Flying Club event where my picture may be taken. Regina Flying Club may use the photo in any manner including but not limited to marketing or advertising and may do minor editing to photos such as cropping from original size or adjusting colour and contrast. There will be no compensation given to the student for use of any photos or media.
